My teenage daughter has gained a lot of weight recently and I’m starting to get worried she will gain so much it affects her health. I try to set a good example by exercising and eating healthy, but she doesn’t follow my example.

It is unusual for a teenage girl not to take more interest in her appearance.
I would suggest a complete medical check-up to be sure there are no health issues involved.
I also would suggest you have a serious talk with her to see if perhaps she has some emotional problem(s). If that seems to be the case, she might benefit from professional counselling.
